COI Disclosure Forms
Annual COI disclosures must be completed electronically using the University's Conflict of Interest Superform website. The Superform website can be used to fulfill only University (i.e., not UPMC/UPP) COI disclosure requirements.

Those with dual appointments at the University and UPMC/UPP can choose instead to complete a combined form available in MyHUB to fulfill their University and UPMC COI reporting requirements at the same time.

To access the combined form, users should log into MyHUB, select the Human Resources tab, and then click the COI Link under My Profile.
