All outside consultants engaged to work on University research projects must complete the disclosure form embedded in University Policy RI02, formerly Policy 11-01-04.
Outside consultants are required to complete a disclosure at the time of appointment, annually thereafter, and at any time during the year if they accrue new outside interests. University employees overseeing work of consultants are ultimately responsible for ensuring that consultants comply with this disclosure requirement.
